How are players' handicaps going to be used in Stroke Play? Add the total allowance to the points earned, or according to the Stroke index, bearing in mind that in several cases a player is disqualified for a hole where he may have scored points, like a ball lost or out-of-bounds?
Johanna, handicaps are applied hole-by-hole according to the Handicap Stroke Table i.e., the player scores points based on his net score. If the player is disqualified from the hole then he receives no points.

In which case would you recommend the 'hole difficulty' or 'match play significance' for courses only wishing to have one Stroke Index table.

At present with so little stableford being played (in the US in particular), the 'difficulty' rating has no significance.
